La mauvaise observance des traitements médicaux par les patients est un problème de santé publique majeur non résolu. Grace au numérique des avancées intéressantes voient le jour. Commençons par définir l’observance thérapeutique. Il s’agit de « l’étude de la cohérence entre le traitement prescrit à un patient, et la capacité de ce dernier à respecter la prescription, tant dans la durée que dans la posologie ». On peut y ajouter « l’adéquation entre le comportement du patient et les recommandations du médecin » assimilées à l’adhérence thérapeutique.

Un patient est observant lorsque son Medication Possession Ratio (MPR), son rapport entre le nombre de jours de traitement délivrés et le nombre de jours de la période de traitement considérée est supérieur ou égal à 80%.

L’Organisation Mondiale de la Santé (OMS) estime que la non-observance des traitements médicaux concerne en France près de 50 % des patients atteints de maladies chroniques.

Des conséquences sur la mortalité…

D’après le Think Tank Fondation Concorde, elle est à l’origine de l’augmentation de la mortalité en France (environ 8000 décès par an) et de nombreuses complications de santé. Et son coût induit deux milliards d’euros par an dont un million en journées d’hospitalisations…

Les Entreprises du médicament (LEEM) donne également des éléments chiffrés sur la non-observance :

• 1 patient sur 2 oublie de prendre ses médicaments
• 1 sur 3 ne suit pas le traitement prescrit
• 1 sur 4 ne respecte pas la dose prescrite
• 3 patients sur 10 ne vont pas jusqu’au bout de leur traitement

Ces derniers chiffres sont à rapprocher d’une autre donnée : au-delà de 3 traitements, situation courante chez les patients atteints de maladies chroniques, la non-observance explose.

• Mieux former les professionnels de santé à la communication sur les traitements
• Créer des outils simples d’usage en consultation et à distance
• Inciter les professionnels à promouvoir l’observance

• Déclarer l’observance “grande cause nationale”

• Un serious game pour motiver les patients : la société californienne Mango Health transpose à l’univers de la santé les codes du jeu pour encourager les patients à mieux suivre leurs traitements. Les utilisateurs gagnent ainsi des points à chaque fois qu’ils prennent correctement leurs médicaments. Ces points leur permettent d’atteindre différents « niveaux ». • Un pilulier connecté intelligent : développé par la startup française Medissimo, Imedipac est un pilulier et une recharge hebdomadaire à préparer soi-même ou par le pharmacien. Il est connecté à la plateforme de santé de Medissimo, pour permettre les échanges avec les professionnels de santé et ses proches. Il enregistre en temps réel les éléments liés à la prise du médicament. Si la plateforme relève un oubli, elle alerte le patient ou son entourage par notification vocale, sms ou mail. Une Version d’imedipac 2 a été développée et intrégre des capteurs capacitifs assurant une détection plus affinée.

• Des applications mobiles: Roche Diabetes Care France a développé l’application Novi-Chek conçue pour amener les jeunes diabétiques entre 13 et 25 ans à s’intéresser par eux-mêmes à leur maladie. Elle les accompagne alors qu’ils sont à une période clé de leur vie de patients diabétiques où il est important d’acquérir les bons réflexes de gestion de la maladie et du traitement. Novi-Chek fournit au patient des informations concernant la maladie et des outils de soutien et de rappels vis-à-vis du traitement (automesure de la glycémie et prise d’insuline après les repas, renouvellement du matériel, …). Ecouter le patient, instaurer le dialogue est un des piliers de la bonne observance. Echanger avec le patient est essentiel pour qu’il appréhende sa maladie, son traitement avec ses bénéfices et ses effets indésirables négatifs, ou encore les modalités d’administration du médicament. Plus le patient sera expert de sa maladie, plus il sera impliqué dans le suivi de son traitement.

Se former pour mieux communiquer sur le sujet et favoriser les échanges avec les patients. Les professionels de santé doivent être capables d’effectuer un transfert de compétences du soignant vers le soigné. Ils doivent maitriser les techniques de communication et tous les aspects pédagogiques de l’éducation thérapeutique du patient. Ils doivent connaitre et recommander les outils digitaux et autres solutions numériques disponibles sur le marché, du pilulier au carnet de suivi, en passant par le programme de télésurveillance et le serious game pour les plus jeunes.

Se coordonner entre les différents professionnels de santé est essentiel pour permettre l’observance d’un traitement. La bonne transmission des informations sur le patient et son/ses traitement(s) entre le médecin spécialiste, le médecin généraliste, le pharmacien ou d’autres professionnels de santé est un enjeu majeur pour résoudre les problèmes d’observance.
